⦁ Is this provider in network or out of network?
⦁ Do I have a co-pay to pay that I will need to pay prior to each visit?
⦁ How many sessions am I able to have each year?
What do I need to have completed prior to each session?
⦁ Your clinician will send you an intake form to complete. Once completed please return documents to the assigned clinician’s secured email.
⦁ Your clinician may send you release authorization forms to sign and return.

What if I don’t have insurance?
Our clinicians offer an hourly rate that may be processed at the time of the session by: Visa, Mastercard or Debit. We also offer a sliding scale income-based rate upon approval of our billing department and managers approval.
